Freshbooks is so easy to utilize. It comes with a clean interface. Its iPhone apps allows for seamless time tracking and billing. I've had this for a number of years and it really fits the bill!

What aspects are problematic or could work better?

I was hoping Freshbooks would let keep time-billing segregated from time tracking. Fortunately, its latest version now allows this.
